About the Office

Evergreen Physical Therapy was founded in 1991 in Mount Vernon, WA, by Jay Smith, DScPT.  We are a Privately Owned Therapy Office that has developed a reputation in the Northwest for our high quality rehabilitation services, fair billing, and by making it our number one priority to put out patients first.

At Evergreen Physical Therapy you will not find the newest and fanciest equipment available.  Instead you will discover a group of professionals with over 90 years of cumulative clinical experience using skills mastered over the years to produce a high level of functional improvement. Still it isn’t just our patients that can tell anybody how well we do, our office is involved in the outcomes collection called “Care Connections.”  This is a program that statistically measures how well our office does with patient services and patient improvement.  In those studies Evergreen Physical Therapy has been rated above the National Network since our participation.
