Evergreen Physical Therapy was founded in 1991 in Mount Vernon, WA, by Jay Smith, PT, DSc, MBA, OCS. We are a Privately Owned Therapy Office that has developed a reputation in the Northwest for our high quality rehabilitation services, fair billing, and making it our priority to put our patients first. At Evergreen Physical Therapy you will not find new, fancy equipment . Instead you will discover a group of professionals with over 100 years of cumulative clinical experience using skills mastered over the years to produce a high level of functional improvement. Still it isn’t just our patients that can tell anybody how well we do, our office is involved in the outcomes collection called “Care Connections.” This is a program that statistically measures how well our office does with patient services and patient improvement. In those studies Evergreen Physical Therapy has been rated above the National Network since our participation. The Northwest Rehab Alliance (NWRA) is network formed from Privately Owned physical therapy clinics.
