Jay graduated from the University of Washington in 1976, with his BSPT degree. During the 80’s, Jay worked for the Skagit Valley hospital with a short leave to pursue a spot as the clinic director for an outpatient office, returning a couple years later to the hospital. During this time Jay also earned his MBA from Western Washington University. He made the leap of faith and opened Evergreen Physical Therapy in early 1991. Since then Jay has mentored numerous undergraduates into PT school, become a Clinical Instructor for the APTA, hosted many interns from every level of education. Became an Orthopedic Certified Specialist (OCS), earned his Doctorate of Science in clinical Physical Therapy (DScPT), a degree above the now standard DPT.
J.R. Smith, DPT
J.R. is the result of a mix of family history and a passion for helping. Originally J.R. earned his PTA degree to feed himself as he pursued his education, earning several Bachelors Degrees and a Masters along the way. However after over 10 years of practice under Jay and the other PT's, his knowledge and skill put him on par with many already established therapists, J.R. applied the University of Washington's Physical Therapy Program and received his Doctorate of Physical Therapy (DPT) in June of 2015. Now he has returned in a capacity to better serve the patients of Evergreen Physical Therapy.
Jeanette Lundberg, PT
Jeanette Lundberg began practicing in 1972 and joined the staff at the Skagit Valley Hospital in 1978. She remained there until joining Evergreen Physical Therapy in 1993. Jeanette is an Aquatic Therapy Instructor and runs the pool program for Evergreen Physical Therapy.
